چکیده

Abstract Background Antibiotic de-escalation (ADE) is a stewardship initiative that aims to reduce exposure antimicrobials, thus limiting their unwanted effect, including antimicrobial resistance. Our study describe the impact of ADE compared with continuation therapy on outcome critically ill coronavirus disease 2019 (COVID-19) patients. Material and Methods A single-center retrospective included COVID-19 adult patients admitted between January 1, August 31, 2021, started broad-spectrum antibiotics. The primary was intensive care unit (ICU) mortality. In addition, other clinical outcomes were evaluated, ICU readmissions, length stay, superinfection. Results 73 mean age 61.0 ± 19.4, performed in 10 (13.6%) these. group, 8/10 (80%) cultures positive. mortality not statistically different groups (60 vs. 41.3%, respectively, P = 0.317). Superinfection occurred 4 (5.4%) Hospital mortality, readmission rates did differ significantly groups. Conclusion De-escalation antibiotics covid-19 associated higher larger cohort needed confirm these findings.

Investigation of the smoking prevalence among critically ill patients with COVID-19

In this paper, we investigate the prevalence rate of smoking in COVID-19 patients and examine whether there is a difference in the distribution of smokers between the two statistical populations of critically ill COVID-19 patients and the entire Iranian population or not. To do this, we first prepared a sample of 300 COVID-19 patients admitted to hospitals in Tehran and Rasht. Then, through the...

Factors influencing the implementation of antibiotic de-escalation and impact of this strategy in critically ill patients

INTRODUCTION A rational use of antibiotics is of paramount importance in order to prevent the emergence of multidrug resistant bacteria that can lead to therapeutic impasse, especially in intensive care units (ICUs). A de-escalation strategy is therefore naturally advocated as part of better antibiotics usage. However, the clinical impact of such a strategy has not been widely studied. We aimed...

Pharmacokinetics Alterations of Midazolam Infusion versus Bolus Administration in Mechanically Ventilated Critically Ill Patients

There is no randomized study carried out in order to compare their pharmacokinetic parameters although midazolam, as a sedative, has been widely administered via continuous infusion as well as intermittent bolus doses in mechanically ventilated critically ill patients. We prospectively investigated the effect of these two principal methods on pharmacokinetic parameters in 23 of mentioned patien...

Antibiotic dose optimization in critically ill patients.

The judicious use of existing antibiotics is essential for preserving their activity against infections. In the era of multi-drug resistance, this is of particular importance in clinical areas characterized by high antibiotic use, such as the ICU. Antibiotic dose optimization in critically ill patients requires sound knowledge not only of the altered physiology in serious infections - including...
